Position Overview

This role seeks licensed Practical Nurses to join the Family Medicine team. Multiple vacancies exist, and applications are accepted continuously. Candidates will provide direct patient care and support the medical team in delivering adequate, compassionate healthcare.

Primary Responsibilities

  • Deliver individualized nursing care to patients of all ages according to physician care plans.
  • Perform vital signs assessment, behavioral health screenings, and routine medical procedures.
  • Manage care coordination, including referrals, order reconciliation, and activity tracking.
  • Educate patients and families on self-care and treatment plans.
  • Participate in organizational efforts to maintain high-quality patient care standards.

Candidate Background

Licensure & Certification:

  • Current, active, unrestricted license as a practical or vocational nurse in a U.S. state, territory, or D.C.
  • Basic Life Support (BLS) certification required; advanced certifications do not replace BLS.

Experience Requirements:

  • At least one year of practical nursing experience in a hospital, clinic, nursing home, or similar medical setting.
  • Experience must include direct patient care, taking vital signs, administering medications or immunizations, and performing routine procedures like EKGs, wound care, and vision or hearing screenings.

Physical & Health Requirements:

  • Moderate to heavy lifting up to 50 pounds.
  • Pre-employment and annual physical examinations.
  • Immunization screening, including mandatory Hepatitis B and seasonal influenza vaccinations.

Other Requirements:

  • Must pass a background check and drug testing.
  • Positions involve interaction with children and youth and require ongoing security verification.
