Position Overview
Signify Health is seeking a dedicated Nurse Practitioner or Physician Assistant for a part-time role conducting In-Home Health Evaluations. This position offers a unique opportunity to engage with health plan members in their homes, providing comprehensive health assessments that help bridge care gaps and connect individuals to the services they need. This is a clinical role focused on evaluation and education, without the responsibilities of prescribing, ordering labs, or altering treatment plans.
Key Responsibilities & Focus Areas
1. Direct Member Engagement & Clinical Evaluation
Travel to and conduct in-home health evaluations, which typically last 45 minutes. Responsibilities include performing a thorough medication and medical history review, completing a physical assessment, and dedicating time to member questions and health education.
2. Diagnostic & Preventive Services
Perform additional diagnostic tests as ordered by the member’s health plan, including diabetic eye exams (DEE), spirometry, bone density screening, and other preventive services.
3. Technology & Workflow Adherence
Utilize a provided iPad and an intuitive, proprietary clinical application to guide and document each evaluation efficiently and accurately.
4. Operational Flexibility & Travel
Be flexible to travel locally within the designated coverage area (Portland, ME, and surroundings) and, if licensed, to other states as needed. This role also includes the potential to conduct virtual visits.
5. Work Environment Note
Clinicians will perform visits in the diverse home environments of CVS members. Candidates must be able to work comfortably and safely in conditions that may include household pets, secondhand smoke, and various airborne allergens.
Candidate Profile: Qualifications & Requirements
Education & Certification Requirements
- Active, unrestricted state license as a Nurse Practitioner or Physician Assistant in Maine.
- Required national board certification (ANCC, AANP, or NCCPA).
- Ability to cover the primary location of Portland, ME, and its surrounding area.
Preferred Qualifications
- Possession of multi-state licenses is highly preferred.
- A compassionate, person-centered approach with excellent interpersonal and communication skills.
- Comfort with technology and mobile clinical tools.
