Role Overview

Signify Health is seeking a full-time nurse practitioner or Physician Assistant to provide In-Home Health Evaluations in the Pittsburgh area. In this role, the clinician engages individuals in their homes, providing comprehensive assessments to improve health outcomes.

Each visit lasts approximately 45 minutes, allowing clinicians to build meaningful one-on-one connections while conducting thorough evaluations. These visits include a detailed review of medical history and medications, a physical assessment, and, when ordered by the health plan, additional diagnostic services such as diabetic eye exams, spirometry, or bone density screenings.

Importantly, clinicians are not required to prescribe medications, order laboratory tests, or modify existing treatment plans. Instead, they focus on assessment, education, and connection to appropriate care resources.

The clinician provides Signify Health services as indicated, including diagnostic and preventive offerings such as spirometry, diabetic eye exams, and bone density screenings. Documentation is completed using an intuitive clinical workflow on an iPad, ensuring accuracy and efficiency.

4. Flexible Field and Virtual Engagement

While primarily conducting in-home visits, the clinician remains flexible to travel locally and within licensed states as needed. Additionally, virtual visits may be conducted when required.

Work Environment

As an in-home clinician, the primary workplace is the private residence of health plan members. These home environments vary and may include household pets, secondhand smoke, or other airborne irritants. Therefore, the clinician must be able to perform duties comfortably and safely in diverse settings.

Compensation and Benefits

The typical salary range for this position is $87,035.00 to $187,460.00 annually. Actual compensation is determined by experience, education, geography, and other relevant factors.
